Investigating synaptic and neuroinflammation biomarkers in the Alzheimer’s disease continuum

Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by complex clinical phenotypes and neuropathological changes. Neuroinflammation and synaptic disruption are emerging as key contributors to AD pathogenesis. This prospective interventional study aims to elucidate the role of these processes in neurodegeneration and predict clinical progression. We will enroll a cohort of 60 subjects diagnosed with mild cognitive impairment (MCI) due to AD. The diagnosis will adhere to 2018 National Institute of Aging and Alzheimer’s Association criteria and include CSF biomarker analysis. Neuroinflammatory markers (GFAP, sTREM2) and synaptic degeneration markers (SNAP-25, GAP-43) will be measured in CSF. Additionally, FDG-PET scans will assess regional brain glucose metabolism. Correlation analyses will reveal associations between biomarker profiles and their contribute to determining neuronal dysfunction, as measured by quantitative FDG-PET. Clinical and neuropsychological longitudinal assessments at 6 and 12 months will investigate biomarker-based predictions of cognitive trajectory. This study will enhance our understanding of neuroinflammation and synaptic disruption in AD and the results may facilitate improvement of diagnostic and prognostic applications of these biomarkers. As disease-modifying therapies emerge, profiling patients based on diverse disease aspects becomes crucial. The findings could contribute to precision medicine strategies for AD. In conclusion, this study seeks to provide valuable insights into the complex interplay of neuroinflammation and synaptic dysfunction in AD. By comprehensively investigating these processes and predicting disease progression, this research aims to contribute to the advancement of AD diagnosis and treatment strategies.

Research activities

My research activity is mainly focused on Alzheimer's disease and related dementias as well as other neurodegenerative diseases. In particular, my PhD project aims to identify valid disease-specific biomarkers of synaptic injury and dysfunction, able to predict the conversion from prodromal Alzheimer's disease to dementia. My interests are also directed towards the study of primary headaches, including migraine and cluster headaches.
