Research activities
My research projects are focused on two specific topics:
1] Role of transorbital sonography for the non-invasive evaluation of intracranial pressure.
Transorbital sonography is a promising tool for the evaluation of intracranial pressure. The basis of the method reside in the variation of intracranial vascular parameters or in the optic nerve sheath diameter (ONSD) that occur during alteration of intracranial pressure. The advantages of using ultrasonography include the non-invasivity of the technique, the ready availability of ultrasonographic machines, rapid performances, relatively low costs, high accuracy and reproducibility of the measurements.
There are crescent evidences of the role of the ONSD for detecting and monitoring elevated intracranial hypertension. Scope of my research project on this issue is to translate the sonographic data in the clinical practice, evaluating the reliability of these methods in patients with intracranial hypertension.
2] Endovascular treatments in acute ischaemic stroke: translation from the benefit of randomised trials to the real life experience
In 2015, the results of five randomised trials comparing medical therapy with mechanical thrombectomy in acute ischaemic stroke have demonstrated the efficacy of endovascular treatment; nowadays, this therapy has been accepted as the standard care in case of large vessel occlusions of the proximal anterior circulation. However, the populations enrolled in trials are more selected than those treated in real world practise and discrepancies in results may be observed. The purpose of my research project is to verify how generalizable are the recent trial findings in the real population through the comparison of the published data with the real life experience. I work as a Neurologist and I have the opportunity to follow the patients admitted to our hospital for acute stroke and I strictly cooperate with radiologists, interventional neuroradiologists, anaesthesiologists, cardiologists, vascular surgeons and other referring figures on stroke network to ensure the best care both in the acute and subacute phase of stroke.
If confirmed, the translation of benefit of endovascular procedures on real life patients could be useful to identify the best therapeutic strategies basing on patients’ characteristics; moreover, positive data could be used for the organization of new regional networks for the acute ischaemic stroke management and pathway.
